This online, dynamic pathway has been developed by a multidisciplinary team of experts to support health care professionals delivering person-centred care for people living with heart failure. Heart failure management is complex, multifaceted and can be delivered in a variety of settings by a range of health care workers. This pathway is designed to support clinician competence and confidence throughout the heart failure journey to increase the quality of guideline directed care.
