Patient initiated follow up

 

Patient initiated follow-up (PIFU) is a new model used in the English NHS to personalise out-patient care. However, the principles of empowering patients to have control over their follow-up care can be applied to any Nation or health system.

A significant amount of work is required before implementing such a model in practice and work needs to be addressed locally to understanding which heart failure patients are likely to benefit, what systems and processes are needed in practice, how to implement PIFU safely and how it will be evaluated are just some of the actions required.
