Treatment with specialist input in the community

Refer to HF MDT/Ambulatory unit/Hospital At home services

 

IV Diuretic Therapy (ALL): If Known diagnosis of HF

Prognostic Therapies

  • Review all heart failure medications and optimise where possible

Other HF interventions:

  • Consider need for advanced care planning/palliative care
  • Referral to cardiac rehab when stable
  • Provide patient education
  • Follow-up with heart failure MDT within 2 weeks of ‘discharge’ from ambulatory care (known HF)
Monitoring (ALL):
  • Assess renal function 24 hourly whilst on IV diuretics
  • Daily Review with assessment of heart rate, blood pressure, renal function, fluid balance and weight until clinically stable for 24-48 hrs
Follow-up
  • For confirmed heart failure cases, consider suitability for remote monitoring if available
  • For suspected new heart failure cases, refer to heart failure assessment service as per NICE guidance
