Psychological support, last year of life and palliation

 

Patients in the last year of life and those with frailty should be identified at an early stage given that:

  1. This is a gateway to virtual wards;
  2. Patients at this stage of life may need deprescribing and a more holistic approach to their care; and
  3. Admission avoidance aligned to an individualised care plan/goals of care plan and comprehensive geriatric assessment and improve QOL.
