Clear follow-up documentation
- Plan for optimising prognostic HF medications (if not already on prior to discharge) - Titration should really be patient specific depending on their need, symptoms and signs (weekly/fortnightly).
- All patients following acute heart failure admission should have a follow up appointment within 2 weeks with a heart failure specialist nurse.
- Plan for frequency of follow-up if stable and if unstable:
a.When do I refer back to a specialist? (i.e. episodes of decompensation, increased HF admissions, reduction in LVEF); and
b.Next routine out-patient/specialist appointment.
